About This Service

Comprehensive GP-Led ADHD Care in Sutherland

Recent reforms in NSW now allow appropriately trained GPs to provide a wider range of ADHD care, including, for a select group of GPs who have completed advanced Tier 2 training, the ability to assess, diagnose, and initiate ADHD medication.

Dr Katrina Mikhail is one of a small number of GPs in NSW to complete this training, making Shire Family Medical one of the few GP practices in the Sutherland Shire able to offer a complete pathway from initial assessment through to ongoing management, without requiring a referral to a psychiatrist or paediatrician.

Dr Katrina Mikhail is one of a select group of GPs in NSW to complete the Tier 2 Endorsed Prescriber training through the NSW Health ADHD in General Practice program. This advanced, competency-based training enables her to assess, diagnose, and initiate ADHD medication, in addition to providing ongoing continuing prescription management for existing patients.

Dr Mikhail has particular clinical interests in ADHD management, paediatrics, chronic health management, preventative care, and women’s health.

How Your Assessment Appointment Works

The initial ADHD assessment is structured across two parts in the same visit, totalling 1 hour and 15 minutes.

Nurse Appointment

You will first meet with our practice nurse, who will review and total your completed questionnaires, take baseline measurements including blood pressure, heart rate, height, weight, and BMI, and perform an on-site ECG.

GP Assessment with Dr Mikhail

A detailed clinical assessment with Dr Katrina Mikhail, covering your history, symptoms across settings, childhood presentation, and current functioning. Dr Mikhail will review your completed questionnaires, discuss findings, and advise on diagnosis and next steps, including whether medication is appropriate.

Outcome & Next Steps

Dr Mikhail will discuss whether ADHD is diagnosed, whether medication is appropriate and which options are available, any further information or assessment required, and your ongoing management plan.

Preparing for Your Appointment

What to Bring to Your Assessment
  • Completed questionnaires — essential, appointment may not proceed without these
  • A written account of how ADHD has impacted you throughout your life. Please prepare this in advance and bring it to your appointment
  • Current medication list (if applicable)
  • Any previous psychological, psychiatric, or allied health assessments
  • Previous school reports or written evidence of symptoms in childhood (adults and children)
  • Hearing and vision test results (children)
  • School-aged children: if possible, a letter from your child's teacher outlining their observations and concerns about ADHD behaviour in the classroom
  • Adults: written collateral history from a family member who can describe their observations of your behaviour and symptoms
  • Any recent blood tests or investigations
  • Medicare card
  • Any other relevant medical or psychological information

School reports matter for adult assessments too. ADHD requires evidence of symptoms across settings and from childhood. Old school reports, if you have them, meaningfully support the assessment, even for adult patients.

Clinical Limitations

When GP-Led Assessment May Not Be Appropriate

GP-led ADHD assessment is not suitable for every patient. The following situations may require assessment by a paediatrician or psychiatrist instead.

  • Complex psychiatric comorbidities requiring specialist-led care
  • Significant diagnostic uncertainty requiring multidisciplinary assessment
  • Situations where non-GP specialist-led care is clinically indicated
  • History of substance use or drug dependence, including medicinal cannabis
  • Children under 6 years of age
  • Patients who may require shared care with a paediatrician or psychiatrist

Attend your assessment

Arrive for your nurse appointment followed by your 45-minute GP assessment with Dr Mikhail. Bring all relevant documents listed in the checklist above.

Recent NSW Health reforms created two levels of GP involvement in ADHD care. Tier 1 (continuation prescribers) can provide ongoing prescriptions for patients who already have a diagnosis. Tier 2 (endorsed prescribers) have completed additional, advanced training that allows them to assess, diagnose, and initiate ADHD medication. Dr Katrina Mikhail is a Tier 2 Endorsed Prescriber — one of a select cohort of GPs in NSW to complete this training.

For children and adolescents, the SNAP-IV and Vanderbilt scales (home and school versions) are essential. For adults, a minimum of one questionnaire from the ASRS, WURS, or DIVA-5 is required. If questionnaires are not completed before your appointment, the assessment may not be able to proceed.

Yes. The ADHD assessment service is available for patients aged 6 and over, including children and adolescents. A parent or guardian must be present throughout the appointment. For children, both the SNAP-IV and Vanderbilt questionnaires are required — including teacher-completed versions — so please allow sufficient time to collect these before your appointment.

It depends on whether you are currently established and stable on ADHD medication. If you have a diagnosis but are not yet on medication, or are not stable on your current medication, please book an ADHD assessment appointment with Dr Mikhail — she can review your situation, assess whether medication is appropriate, and initiate treatment where indicated. If you already have a confirmed diagnosis and are stable on an existing medication, you may be eligible for our continuing prescriptions service instead. You will need a current correspondence letter from your psychiatrist or paediatrician and must be registered for MyMedicare at our practice.
